Make a request
Write to the Practice Manager, Uxendon Crescent Surgery, 92 Preston Road, Wembley, HA9 8NL. Include your name, an address for correspondence and a description of the information you want. We normally respond within 20 working days. Some information may be exempt from disclosure.
Ask before ordering copies if charges may apply. Viewing these website pages is free.
Your own medical records
Use the health record request route for your personal information. Do not use Freedom of Information to request someone else’s confidential records.
