Feedback and complaints
Tell us about your experience
We welcome suggestions, concerns and compliments. Speak to reception or phone 020 8904 3883. Ask about the Friends and Family Test or patient participation group.
Make a complaint to the practice
Ask to speak to the practice manager, or write to: Practice Manager, Uxendon Crescent Surgery, 92 Preston Road, Wembley, HA9 8NL. You can make a complaint verbally; it does not have to be in writing.
Explain what happened, when it happened and what you would like us to look into. Tell us if you need help communicating your complaint.
Other routes and support
You may instead complain to NHS West and North London ICB as commissioner. It is not a second investigation route for the same complaint after the practice has responded.

If you remain dissatisfied after the local complaints process, contact the Parliamentary and Health Service Ombudsman. Independent NHS complaints advocacy can help you raise concerns. Ask your local council or Healthwatch about support.
Time limits and representation
Normally complain within 12 months of the event or of discovering the problem. Explain any reason for delay; exceptions may be possible. If you complain for someone else, we will check consent or your authority to act.
