Off work for 7 days or less

You normally self-certify with your employer. Count weekends and bank holidays in those 7 days.

Off work for more than 7 days

Ask the healthcare professional treating you about a fit note. To request one from the practice, contact us online or phone reception. A clinician will assess whether a fit note is appropriate.

For an extension, tell us when your current note ends and what has changed. Sending a request does not guarantee a note will be issued.
